Adult Day Services offered through the HCBS Day Habilitation Waiver is an organized day activity program designed to assist people in becoming more involved in the community.  The program also teaches skills to aid in becoming more independent or maintaining the level of independence already achieved by the individual.

 

Activities which may be offered in Adult Day Services:

  • Crafts & Hobbies

  • Current Events

  • Community Outings

  • Music & Art Appreciation

  • Social Skills

  • Program Management

  • Health, First Aid, & Safety

  • Household Management

  • Self Advocacy

  • Human Awareness

Benefits:

Adult Day Services offers you the choice not to work.  Whether you are ready to retire, physically or mentally unable, or you simply do not wish to work, the Adult Day Services program offered by Concerned, Inc. is right for you. 

Additional Services to provide adults who wish to continue living independently with alternatives to facility-based, long-term care.  The following services are provided to the general aging population in conjunction with the Case Management Program through Southwest 8 Area Agency on Aging.  Financial assistance is available to qualifying individuals through the Medicaid Home and Community Based Elderly Waiver, the Senior Living Trust Fund, and the Family Caregiver Dollars Program.

  • In-Home Respite Care - Respite care services are temporary care to a participant designed to provide relief to the usual informal caregiver.  This service is provided  by Aging Support staff trained in eldercare issues including Alzheimer’s Disease.

  • Assistive Devices - This service provides practical equipment to assist persons with activities of daily living, such as: shower chairs, transfer boards, dressing aides, adaptive eating equipment.

  • Home and Vehicle Modifications - These services are physical modifications made to the consumer’s home or vehicle used for transportation which allow the consumer more independence, such as: bathtubs and toilets to accommodate wheelchair transfer, entrance ramps and rails, and lifts for porches and stairs.
