OUTCOMES MEASUREMENT REPORT – 2011

Concerned, Inc. provides training
and supports to people with
disabilities in the following areas:
Organizational Employment,
Community Employment, Adult Day
Services and SCL/Hourly and
Residential Services.
Services in Organizational
Employment, HCBS Residential, and
Adult Day Services are provided in
Harlan. Community Employment
Services and HCBS Hourly Services
are provided primarily in the Harlan
area, although Community Employment
has developed jobs in neighboring
communities and Hourly Services can
be provided anywhere that the
individual receiving the service
chooses.
Concerned, Inc. is located at
1812 Industrial Parkway in Harlan,
Iowa.
The 52,500 square foot
building, where we are located has
been occupied since April 2005.
The purchase of the space has
allowed Concerned, Inc. to increase
and maintain a client population of
102 in Organizational Employment and
29 in Adult Day Services.
Our ultimate goal is to
become a financially solvent agency.
With our current location we
are able to continually increase the
number of clients we serve and the
services we can offer, thus showing
an increase in revenue.
Our mission is held in constant
priority by the staff, Executive
Director and Board of Directors.
In 2011 we, as an agency
performed very well meeting the
following list of accomplishments:
*Passed IME audit with
significant improvements from the
last one
*Awarded a 3 year
accreditation from CARF (our 6th
3-year accreditation)
*Successful DIA accreditation
with a 2-year accreditation awarded
*5% cost of living increase
given out to employees
*ADS and Org. Employment
running at record high numbers
* Most successful golf
tournament since 1998
*Celebration of 40 years in
business
Concerned, Inc. identified our
agency goals for 2011 in our
Accessibility Plan.
The first goal was to update
all marketing materials to describe
our Org. Employment program as a
“Job Skills Training Center.”
This was completed by
September 2011.
The second goal for 2011 was
to install sidewalks on the north
and east sides of our building and
to make all our agency owned homes
wheelchair accessible.
Much research and
conversations took place in regards
to the sidewalk issue.
We are unable to sustain the
cost and the city of Harlan is not
willing to get involved in the
project, so we have made flags to be
used to staff to stop traffic in the
event of a building evacuation.
All of our homes have been
made wheelchair accessible except
one.
That accessibility of that
house is being addressed in our 2012
plan.
The sprinkler heads needed
per our Fire Marshall report were
installed in March 2011.
Concerned, Inc. has met the goals
set for 2011.
We will report on the
following goals for 2012 which are
identified in our accessibility and
strategic plan.
1.)
Making our one remaining agency
owned house wheelchair accessible.
2.)
Making our client entrance more user
friendly with the installation of a
wheelchair remote access button and
the installation of a removable
breezeway to use during the winter
months.
3.)
Re-routing of our NCU computer units
so that they run more efficiently
and effectively as they are now
often out of operation.
4.)
Public Recognition of the best
businesses in the community that
treat our clients well.
SUMMARY
Concerned, Inc. continues to
demonstrate a customer-driven
philosophy.
Client choice and
individuality are the primary
aspects of providing quality
services to the people we serve.
We will continue to explore
new services to provide, in the most
cost-effective manner, yet always
keeping our mission and values in
the forefront of our agency’s
philosophy.
We are aware of our strengths
and also where our weaknesses lie.
We are very proud of the
accomplishments we have already
achieved and look forward to future
possibilities for growth and
expansion.
