CONCERNED, INC.

OUTCOMES MEASUREMENT REPORT – 2007

Concerned, Inc. provides training and supports to people with disabilities in the following areas:  Organizational Employment, Community Employment, HCBS/Residential and Adult Day Services.  Services in Organizational Employment, HCBS, and Adult Day Services are provided in Harlan and Community Employment Services are provided primarily in the Harlan area, although we have begun to develop jobs in neighboring communities.

Concerned, Inc. is located at 1812 Industrial Parkway in Harlan, Iowa.  We purchased the 52,500 square foot building in 2004 using grants, USDA loans and public donations.  Concerned, Inc. has bee occupying the building since April 2005.  The purchase of the space has allowed Concerned, Inc.  to increase and maintain our client population of 90 in Organizational Employment and 9 in Adult Day Services.  Our ultimate goal is to become financially solvent agency.  With our current location we are able to continually increase the number of client we serve and services we can offer, thus showing an increase in revenue.

The mission of Concerned, Inc. is to empower people to obtain their highest level of independence.  Our mission is held in constant priority by the staff, Executive Director and Board of Directors.

As an agency, Concerned, Inc. identified short and long range goals for 2007.  Our first goal was finding other means to create additional income for the agency and work for our clients.  Concerned, Inc. met this goal by starting an enterprise business that specializes in personalized napkins, wedding programs, invitations, Christmas cards, etc.  We have also obtained fourteen new or different contracts for our clients.  This goal will always be ongoing for our agency as we will always need additional income and work. Goal 2 was not met in 2007.  We are going to continue this goal for 2008.  The income from programs can substantiate wage increase and benefits for staff if they did not have to fund building and home improvements, capital purchases, etc.  We must improve our fundraising monies and our foundation monies.  Exploring the possibility of providing transportation for our clients after hours when public transportation is not available was goal 3.  Concerned, Inc. has met this goal by purchasing 3 agency vehicles.  However with doubling our residential program in the last 1.5 years we continue to struggle with not enough vehicles for everyone to use.  Concerned, Inc. works hard to coordinate the use of the vehicles that we have but there are times when coordinating the vehicles we have is not enough.  Goal 4 of educating the public on the services that Concerned, Inc. provides was met.  We have a Marketing Plan and a Marketing Director that completes this goal for us.  Concerned, Inc. has an article in the local newspaper at lease once a month.  Many of the agency’s Administration staff and Program Coordinators will speak to local groups if asked.  We advertise on our local radio station (KNOD), and provide tours to anyone or group that is interested.  Concerned, Inc. holds an annual information night.  Invites to areas schools, fenders, stakeholders, and the public are sent out by letters and in the local newspaper.  Making the south fire door accessible to all clients including wheelchairs, as goal 5, was not completed.  This was due to not enough funds in our budget and until we can establish these funds this goal will not be completed.  It is still and area we will pursue in the future.  As an agency Concerned, Inc. identified short and long range goals from our accessibility and our strategic plan.  The team identified these areas to report on for 2007.

1.)      Increase staff wages and benefits to decrease staff turnover and hire qualified staff.

2.)      Increase our foundation base to off set capital purchases and other project needs.

3.)      Educate the public on the services that Concerned, Inc. provides and promote our new enterprise business.

4.)      Recruit clients to increase growth in all programs.

5.)      Purchase a ten pallet straight truck for production use.  By purchasing the truck we will be able to increase our work contracts.

SUMMARY

Concerned, Inc. continues to demonstrate a customer-driven philosophy.  Client choice and individuality are the primary aspects of providing quality services to the people we serve.  With the addition of our building and new services offered we have progressed in meeting those individual’s specific needs.  We will continue to explore new services to provide, in the most cost-effective manner, yet always keeping our mission and values in the forefront of our agency’s philosophy.  We are aware of our strengths and also where our weaknesses lie.  We will again utilize the information we gather and the input from our customers to improve the way we provide services to them.  We are very proud of the accomplishments we have already achieved and look forward of future possibilities.
